TL;DR

Short answer: for a clinic or group practice, the best cybersecurity software starts with Microsoft 365 Business Premium at $22 per user a month, which Microsoft covers under its HIPAA business associate agreement by default. Add Paubox for encrypted patient email, Huntress Managed EDR for a 24/7 team on the workstations and Accountable for the risk assessment and policies.

For a 25-person clinic, that core stack plus KnowBe4 training comes to $1,335.75 a month at list prices, roughly $53 per staff member. A Nessus Professional licence for the scans HHS has proposed adds $4,790 a year.

  • Updated: October 24, 2026
  • Top pick: Microsoft 365 Business Premium (best for: Practices already on Outlook that need MFA, device management and email filtering under one BAA)
  • Top pick price as of October 24, 2026: Microsoft 365 Business Premium: $22/user/mo paid yearly; HIPAA BAA offered by default
  • 9 tools compared: Microsoft 365 Business Premium, Huntress Managed EDR, Paubox, Accountable, Cisco Duo, Tenable Nessus Professional, Virtru, KnowBe4 Security Awareness Training, Egnyte
  • Huntress Managed EDR (best for: Clinics whose IT support works office hours and cannot watch workstations at night): $8.99/endpoint/mo MSRP; no minimum through an MSP, 50 seats direct
  • Paubox (best for: Practices that email patients and referring offices daily and want encryption with no portal for the recipient): Standard $32/mo for up to 5 senders, billed annually; BAA with every account
  • Accountable (best for: Practice managers who own HIPAA compliance and need a risk assessment, policies and BAA tracking in one place): Basic HIPAA $169/mo billed annually for 15 employees; extra seats $9/mo

Toolradar data: the 865 security tools in Toolradar's October 2026 security ranking (Toolradar is the software directory we run) were the starting pool. Nine made this list because a clinic without a security team can run or outsource them, and each is priced for the same 25-person practice from vendor pages read on 17 October 2026.

The rules are moving. In January 2025, HHS published a proposed HIPAA Security Rule update that would expressly require multi-factor authentication, encryption of all ePHI at rest and in transit, vulnerability scans at least every six months and written procedures to restore critical systems within 72 hours. As of October 2026 no final rule has appeared in the Federal Register.

Buying for the proposal now costs little more than buying for today's rule, and it means the practice is not re-tendering its stack the year the final text lands. Recent breaches we covered, such as the Oracle Health Cerner incident and the Aesto Health breach, show what is at stake.

1 Microsoft 365 Business Premium logo
Top Pick

Microsoft 365 Business Premium

Entra ID P1, Intune, Defender for Business, Defender for Office 365 and Purview in one licence covered by Microsoft's BAA.

Pricing
$22/user/mo paid yearly; HIPAA BAA offered by default

Best for: Practices already on Outlook that need MFA, device management and email filtering under one BAA

Pros

  • Microsoft offers its HIPAA BAA by default to covered entities and business associates, so the base platform needs no separate agreement negotiation.
  • Intune Plan 1 can remotely wipe a lost clinician laptop or tablet, the device most often behind a reportable loss.

Cons

  • Microsoft says plainly that its BAA does not make a practice compliant; the policies, conditional access and encryption settings are the practice's job.
  • Defender for Business detects threats but nobody responds overnight unless a managed service sits on top.
2 Huntress Managed EDR logo

Huntress Managed EDR

A human-led SOC on every workstation around the clock, with remediation and managed antivirus in the rate.

Pricing
$8.99/endpoint/mo MSRP; no minimum through an MSP, 50 seats direct

Best for: Clinics whose IT support works office hours and cannot watch workstations at night

Pros

  • The 24/7 SOC investigates and stages remediation around the clock, so ransomware started on a Saturday is contained before Monday clinic hours.
  • Huntress sells a dedicated healthcare offering and an identity add-on, Managed ITDR at a $4.80 MSRP, for Microsoft 365 account takeover.

Cons

  • Buying direct or from a reseller triggers a 50-seat floor per product, so a 25-workstation clinic saves by going through an MSP.
  • Huntress does not publish whether it signs a BAA, so ask before deployment if its agents can reach systems holding ePHI.
3 Paubox logo

Paubox

HIPAA email encryption on Google Workspace or Microsoft 365, with inbound protection and DLP on higher tiers.

Pricing
Standard $32/mo for up to 5 senders, billed annually; BAA with every account

Best for: Practices that email patients and referring offices daily and want encryption with no portal for the recipient

Pros

  • Every account includes a signed BAA, and the service is HITRUST certified, which shortens the vendor review for a small practice.
  • Plus at $65 a month for 5 senders adds inbound filtering against spam, ransomware and phishing, and Premium at $75 adds archiving and data loss prevention.

Cons

  • Pricing scales by sender, so a clinic where all 25 staff email patients pays $242 a month on Standard under the vendor's calculator.
  • Monthly billing costs more than annual; the Standard base pack is $41.60 a month without the yearly commitment.
4 Accountable logo

Accountable

HIPAA compliance software with a security risk assessment, policies, staff training, incident response and BAA management.

Pricing
Basic HIPAA $169/mo billed annually for 15 employees; extra seats $9/mo

Best for: Practice managers who own HIPAA compliance and need a risk assessment, policies and BAA tracking in one place

Pros

  • Basic includes the security risk assessment, policies and procedures, HIPAA training and a vendor and BAA register, the documents an HHS investigation asks for first.
  • Annual billing costs $2,028 a year versus $199 a month billed monthly, and Accountable lets monthly plans cancel anytime.

Cons

  • It documents the program rather than blocking attacks, so budget it beside the technical tools, never as a replacement for them.
  • Plus at $254 a month adds monitoring and deeper vendor management, and hands-on help starts on Pro at $679.
5 Cisco Duo logo

Cisco Duo

Phishing-resistant MFA and SSO in front of clinical apps, marketed by Cisco for HIPAA and EPCS use.

Pricing
Essentials $3/user/mo; Advantage $6; Premier $9; free edition for 10 or fewer users

Best for: Practices that need MFA on the EHR, VPN or remote desktop outside Microsoft's sign-in

Pros

  • With Essentials, phishing-resistant MFA and passwordless login reach the clinical apps Microsoft sign-in does not, which is where the proposed MFA requirement bites.
  • Duo pitches telehealth and e-prescribing (EPCS) access, so one MFA prompt can protect the patient portal and the prescribing workflow.

Cons

  • A practice already on Business Premium has Entra ID P1 for MFA, so Duo is a second identity system to run unless the EHR needs it.
  • Device trust and identity threat detection sit on Advantage and Premier, two to three times the Essentials rate.
6 Tenable Nessus Professional logo

Tenable Nessus Professional

A licensed vulnerability scanner with unlimited assessments, configurable reports and real-time vulnerability updates.

Pricing
$4,790 for a 1-year licence; $9,330.95 for 2 years

Best for: Practices or their IT partner that want to run the twice-yearly vulnerability scans themselves

Pros

  • Unlimited vulnerability scanning on a single annual licence lets the practice scan every six months, or monthly, without per-asset fees.
  • Configurable reports give the annual risk assessment a dated record of what was found and fixed, which is the evidence an investigator asks to see.

Cons

  • The yearly licence is the second-largest line in a small clinic's budget after Microsoft 365, so many practices buy scanning from their MSP instead.
  • It finds problems but does not patch them; someone still has to schedule fixes on clinical workstations without disrupting care.
7 Virtru logo

Virtru

End-to-end encrypted email and attachments for Microsoft 365 or Google Workspace, with a signed BAA on Starter.

Pricing
Starter $119/mo for 5 users, billed annually; signed BAA included

Best for: Practices that want encrypted email with revoke and expiry controls for a small group of senders

Pros

  • Starter ships with a signed BAA, and patients read the encrypted message without creating an account.
  • Expiry dates and after-the-fact revocation help when a lab result reaches the wrong patient.

Cons

  • Five users cost $119 a month, several times what Paubox Standard charges for the same five senders.
  • Seats are sold in packs of five with extra users priced on request, so covering the whole clinic means talking to sales.
8 KnowBe4 Security Awareness Training logo

KnowBe4 Security Awareness Training

Phishing simulation and awareness training priced per seat, with Foundation and Advanced content tiers.

Pricing
Foundation $2.40/seat/mo at 25-50 seats, three-year term

Best for: Clinics that want phishing simulations for front-desk and billing staff on a published per-seat rate

Pros

  • A 25-person clinic sits exactly at the bottom of the smallest band, so it pays the published rate without buying seats nobody uses.
  • Advanced at $3.75 a seat adds a larger content library for practices that want more than annual training.

Cons

  • The rate is for a 3-year term, so the practice commits $2,160 at 25 seats before the first campaign.
  • Accountable's Basic plan already includes HIPAA training, so check the overlap before buying both.
9 Egnyte logo

Egnyte

File sharing with HIPAA-ready compliant storage, ransomware detection and suspicious login alerts on Enterprise Lite.

Pricing
Business $22/user/mo; Enterprise Lite $39/user/mo, both paid annually

Best for: Practices and billing offices sharing large files with outside providers who need governed access

Pros

  • On Enterprise Lite, Egnyte watches shared folders for known ransomware artifacts and flags unusual logins from outside partners.
  • Compliant storage options list HIPAA alongside SOC 2 and ISO 27001, useful for imaging centres and billing companies with outside partners.

Cons

  • Enterprise Lite costs more per user than Business Premium, so it pays off only where SharePoint and OneDrive do not fit the workflow.
  • Recovering from snapshots and classifying sensitive records both require Ultimate, a quote-only tier.

What it is

Cybersecurity software for healthcare protects electronic protected health information (ePHI) across sign-in, workstations, email, files and staff behaviour, from vendors willing to sign a business associate agreement (BAA) when they handle that data. The BAA is the filter most generic security lists skip.

A vendor that stores, transmits or can access ePHI is a business associate, so the practice needs that signed agreement before patient data touches the service. Microsoft says it offers its HIPAA BAA by default through its data protection terms, and Paubox includes one with every account. A BAA alone does not make a practice compliant, a point Microsoft makes on the same page.

The jobs map to tools: Microsoft 365 for identity, devices and mail filtering, Huntress for endpoint response, Duo for MFA in front of the EHR and VPN, and Nessus for the vulnerability scans.

Why it matters

A clinic's downtime is measured in cancelled appointments, not lost files. That is why the proposed 72-hour restore requirement matters more to a practice manager than any feature list: if the EHR workstation and scheduling system are down for a week, the revenue gap dwarfs the cost of every tool on this page.

The second pressure is the paperwork. HHS investigations look for a written risk analysis, policies and signed BAAs, and the proposal adds a technology asset inventory and a network map. Software that produces those documents, such as Accountable, saves a practice from hiring a consultant each year.

The third is email. Referrals, lab results and billing questions travel by email, and a plain message to a patient's personal inbox is the easiest HIPAA mistake to make. For the controls that hold when someone clicks a bad link anyway, see how to prevent phishing attacks.

Key features to look for

A signed BAA
Any service that stores, sends or can reach ePHI needs one. Microsoft offers it by default, Paubox and Virtru include it, and for every other vendor ask before the trial ends.
MFA everywhere the proposal names
The HHS proposal would expressly require multi-factor authentication. Check it covers the EHR, remote access and email, not only Microsoft 365 sign-in.
Scans on a schedule
Vulnerability scans at least every six months are in the proposal. A licensed scanner and a calendar reminder are cheaper than a consultant's one-off report.
Who answers the 2 a.m. alert
Practices rarely have IT staff on call. A managed EDR service puts a human on the alerts, which is the line that keeps a weekend intrusion from becoming a Monday outage.

Pricing

Every figure below is a US dollar list price or MSRP read on the vendor's pricing page on 17 October 2026; taxes, MSP fees and negotiated discounts are not included. The scenario is a 25-person outpatient clinic with 25 workstations, where every staff member emails patients or partners.

The core stack comes to $1,335.75 a month, or $16,029 a year: Microsoft 365 Business Premium $550, Accountable Basic with 10 extra seats $259, Paubox Standard for 25 senders $242, Huntress Managed EDR through an MSP $224.75 and KnowBe4 Foundation $60. Nessus Professional adds its yearly licence, about $399 a month spread over twelve months.

Bought direct, Huntress's 50-seat floor lifts its line to $449.50. Paubox's 25-sender figure comes from the calculator on its pricing page: the 5-sender base plus a per-sender rate for senders 6 to 50.

Methodology: we started from Toolradar's October 2026 security ranking, kept tools a practice without security staff can run or outsource, and ordered them by the controls named in the HHS proposal. Dupple has no test lab, so this page relies on vendor pages and the Federal Register, not hands-on testing. BAA status is stated only where the vendor publishes it.

Mistakes to avoid
×Signing up for a tool before checking the BAA. A free trial that receives a patient's email is already a business associate relationship.
×Treating the Microsoft BAA as compliance. Microsoft states that using its services does not on its own achieve HIPAA compliance.
×Leaving MFA on Microsoft 365 only. The EHR, the VPN and remote support tools are the logins attackers try next.
×Running one vulnerability scan for the annual risk assessment and never again. The proposed rule asks for scans at least every six months.
Expert tips
→Keep a one-page register of every vendor that touches ePHI, with the date its BAA was signed. Accountable's BAA management does this if you buy it.
→Ask your MSP whether it resells Huntress and runs scheduled Nessus scans. Bundled through an MSP, both can cost less than buying them separately.
→Price Paubox by the number of real senders. If only five staff email patients, the clinic pays the base pack, not the 25-sender rate.
→Write the 72-hour restore plan now: which workstation runs the EHR client, where its backup sits and who restores it on a Sunday.
→Run the first KnowBe4 phishing test on the front desk and billing team, who receive the most outside email.

The bottom line

Start with Microsoft 365 Business Premium, configured with MFA and device policies, because its BAA covers the platform most practices already use. Add Paubox for patient email, Accountable for the risk assessment and policies, and Huntress through an MSP so someone answers alerts when the clinic is closed.

Budget for the proposed Security Rule now: MFA beyond email, encryption, twice-yearly scans and a 72-hour restore plan. For a 25-person clinic the core stack runs about $53 per person a month at list prices. Frequently asked questions

What is the best cybersecurity software for healthcare in 2026?

For most clinics and group practices, Microsoft 365 Business Premium is the base, because it bundles MFA, device management and email filtering and Microsoft offers its HIPAA BAA by default. Add Paubox for encrypted patient email, Huntress Managed EDR for 24/7 response and Accountable for the HIPAA risk assessment. Prices were checked on vendor pages in October 2026.

How much does HIPAA cybersecurity cost for a small clinic?

At October 2026 list prices, a 25-person clinic running Microsoft 365 Business Premium, Accountable Basic, Paubox Standard for 25 senders, Huntress through an MSP and KnowBe4 pays $1,335.75 a month, about $53 per person. A Nessus Professional licence for vulnerability scanning adds $4,790 a year. MSP labour is extra.

Does a security vendor need to sign a BAA?

Yes, if it creates, receives, maintains or transmits ePHI for the practice, it is a business associate under HIPAA and needs a signed BAA. Microsoft offers one by default for in-scope services, Paubox includes one with every account and Virtru includes one on Starter. Ask any other vendor before patient data reaches it.

What does the proposed HIPAA Security Rule change?

The HHS proposal published on 6 January 2025 would expressly require multi-factor authentication, encryption of all ePHI at rest and in transit, vulnerability scans at least every six months, a technology asset inventory with a network map, and procedures to restore critical systems within 72 hours. As of October 2026 no final rule has been published in the Federal Register.

Is Microsoft 365 HIPAA compliant?

Microsoft offers a HIPAA BAA by default for in-scope cloud services, including Exchange Online and SharePoint Online, but it states that using its services does not on its own achieve compliance. The practice still has to configure MFA, encryption, access policies and audit logging, and keep its own risk analysis.

What is the cheapest HIPAA-compliant email?

Among the vendors priced here, Paubox Standard is the cheapest at $32 a month for up to 5 senders billed annually, with a BAA included. Virtru Starter covers 5 users for $119 a month. Microsoft 365 Business Premium also includes email under Microsoft's BAA, but patients without a secure portal need message encryption configured.
